1. You must immediately inform the GDC if you return to the UK to provide dental services which require GDC registration. 2. You must notify the GDC within seven days of any post you accept for which GDC registration is required, and the Commissioning Body on whose Dental Performers List you are included. 3. If employed, you must provide the contact details of your employer(s) to the GDC within seven days and allow the GDC to exchange information with your employer(s) or any contracting body/bodies for which you provide dental services. 4. You must inform the GDC within seven days of any formal or informal disciplinary proceedings taken against you, from the date of this determination. 5. You must inform the GDC within seven days of any complaints made against you from the date of this determination. 6. You must inform the GDC within seven days from the date of application if you apply for dental employment outside the UK. 7. At any times you are present at your place(es) of work, where practice staff (save for the chaperone(s) approved by the GDC) or the public are present, you must have a chaperone with you at all times. You must provide the name and full contact details of any proposed chaperone(s) to the GDC within seven days. The chaperone is to be a GDC registrant and is to be approved by the GDC. The chaperone must submit signed confirmation to the GDC confirming that they consent to act in this role. 8. You must allow the GDC to exchange information about your conditions with any chaperone. 9. You must ensure that your chaperone is notified of these conditions. You must provide evidence of this to the GDC within seven days of the chaperone being approved. 10. You must ensure that every member of the dental team, employee(s) and any dental students at the practice(s)/hospital(s)/clinic(s) where you work are notified of these conditions, and provide evidence that this has been done within 14 days. 11. You must permit other members of the dental team, employee(s) and any dental students at the practice(s)/hospital(s)/clinic(s) where you work and/or your chaperone to contact the GDC immediately if they are concerned about your fitness to practise and/or your compliance with these conditions. 12. At any time you are providing dental services which require you to be registered with the GDC, you must agree to the appointment of a reporter nominated by you and approved by the GDC. The reporter shall be a GDC registrant in the same category of the register as you. The reporter shall work in the same practice as you. 13. The chaperone and the reporter could be the same person. 14. You must provide a report from the reporter to the GDC every two months and at least 14 days prior to any review hearing. This report should include: i. Confirmation of compliance from the chaperone(s) with condition (7) above; ii. Confirmation of compliance with these conditions; iii. Details of any complaints received and their resolution; iv. Any other relevant information. 15. You must allow the GDC to exchange information about your conditions with any reporter. 16. You must inform within one week the following parties that your registration is subject to the conditions, listed at (1) to (15), above, and provide written evidence to the GDC within seven days from the date of this determination: a. any organisation or person employing or contracting with him to undertake dental work; b. any prospective employer (at the time of application); c. all members of staff at your place of work; and, d. any Commissioning Body in whose Dental Performers List you are included or seeking inclusion (at the time of application). 17. You must permit the GDC to disclose the above conditions, (1) to (16), to any person requesting information about your registration status.
