1. He must notify the GDC within 7 days of any post he accepts for which GDC registration is required and the Commissioning Body on whose Dental Performers List he is included. 2. If employed, he must provide the contact details of his employer(s) to the GDC within 7 days and allow the GDC to exchange information with his employer(s) or any contracting body/bodies for which he provides dental services. 3. He must inform the GDC within 7 days of any formal or informal disciplinary proceedings taken against him, from the date of this determination. 4. He must inform the GDC within 7 days of any complaints made against him. 5. He must inform the GDC within 7 days from the date of application, if he applies for dental employment outside the UK. 6. At any time he is providing dental services, which require him to be registered with the GDC, he must agree to the appointment of a workplace reporter(s) nominated by him and approved by the GDC. The reporter(s) shall be a GDC registrant in the same category as the registrant (or higher). The reporter(s) shall be on the premises at any time that the registrant is there. 7. He must permit other members of the dental team, employee(s) and any dental students at the practice(s) /hospital(s)/clinic(s), where he works to contact the GDC immediately if they are concerned about his fitness to practise and/or compliance with these conditions. 8. He must provide a report from the reporter(s) to the GDC every 2 months and at least 14 days prior to any review hearing. This report should include: I. Confirmation of compliance with these conditions; II. Details of any complaints received and their resolution; III. Any other relevant information. 9. He must allow the GDC to exchange information about his conditions with any reporter(s). 10. He must not engage in single-handed dental practice. 11. He must not provide dental services work as a locum or out-of-hours. 12. He must inform within 1 week the following parties that his registration is subject to the conditions, listed at (1) to (11) above and provide written evidence to the GDC within 7 days from the date of this determination: a. any organisation or person employing or contracting with him to undertake dental work; b. any prospective employer (at the time of application); c. all members of staff at his place of work; and, d. any Commissioning Body in whose Dental Performers List he is included or seeking inclusion (at the time of application). 13. He must permit the GDC to disclose the above conditions, (1) to (12), to any person requesting information about his registration status.
