1. You must notify the GDC within 7 days of any professional appointment which you currently have, or which in the future you accept, and provide the contact details of your employer or any organisation for which you are contracted to provide dental services. 2. You must allow the GDC to exchange information with your employer or any organisation for which you are contracted to provide dental services, or any Workplace supervisor referred to in these conditions. 3. You must inform the GDC within 7 days of any informal or formal disciplinary proceedings taken against you from the date these conditions take effect. 4. You must inform the GDC within 7 days of any complaints made against you from the date these conditions take effect. 5. You must inform the GDC within 7 days if you apply for dental employment outside the UK. 6. You must inform the GDC, within seven days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within seven days of making an application for registration with any overseas regulator or equivalent authority. 7. At any time you are employed as a Dental Practice Manager, you must agree to the appointment of a workplace reporter nominated by you and approved by the GDC. The reporter must be a GDC registrant in the same category of the register or higher. The reporter must work at the same practice. 8. You must not work as a Dental Practice Manager, until a proposed workplace reporter has been approved by the GDC. 9. If and when working as a Dental Practice Manager, you must provide reports from the workplace reporter to the GDC every three months and at least 14 days prior to any review hearing. The workplace Reporter’s report should address the following areas: • Dates of meetings between you and the reporter • Confirmation that you are complying with your Conditions • Details from your reporter outlining any concerns about your fitness to practise • Any other information relevant to your compliance with your conditions • Any areas of concern. 10. If and when working as a Dental Practice Manager, you must present the reporter with a copy of this determination within 7 days of the reporter being approved. You must provide the GDC with written evidence that you have notified the reporter within 7 days. 11. At any time you are employed as a Dental Practice Manager, you must remain under the close supervision* of a workplace supervisor nominated by you and approved by the GDC. The workplace supervisor must be a GDC registrant in the same category of the register or higher. 12. You must not work as a Dental Practice Manager, until a proposed workplace supervisor has been approved by the GDC. 13. If and when you are working as a Dental Practice Manager, you must provide reports from the workplace supervisor to the GDC every three months and at least 14 days prior to any review hearing. The workplace supervisor’s report should address the areas specified in Condition 17. 14. If and when working as a Dental Practice Manager, you must present the Workplace Supervisor with a copy of this determination within 7 days of the Workplace Supervisor being approved. You must provide the GDC with written evidence that you have notified the Workplace Supervisor within 7 days. 15. A Workplace Supervisor and Reporter can be the same person. 16. If working as a Dental Practice Manager, you must maintain a log detailing the daily checks of essential medical kit, in particular, the log should confirm: - Medical emergency drugs (such as Buccal Midazolam, Glucagon) are regularly checked, in date, and readily accessible. - Medical emergency equipment (such as clear face masks for self-inflating, paediatric adhesive defib pads, and self-inflating bag valve apparatus for both adults and children) are regularly checked, in date, and readily accessible.

17. You must provide a copy of these logs to the GDC every three months and at least 14 days prior to any review hearing, or, alternatively, confirm why you have not completed such logs that period. The logs must be signed by the Workplace Supervisor. 18. You must not work as a locum Dental Practice Manager, or undertake any out-of-hours work or on-call duties as a Dental Practice Manager. 19. You must inform within 7 days the following parties that your registration is subject to these conditions and provide evidence to the GDC that this has been done: a) Any organisation or person employing or contracting with you to undertake dental work; b) Any prospective employer (at the time of application); c) Every member of the dental team and all employees at the practice(s) where you work. They must also be informed that they should contact the Reporter / Workplace supervisor and the GDC immediately if they are concerned about your fitness to practise and/or your compliance with the conditions for the Reporter / Workplace Supervisor to include within their report. 20. You must permit the GDC to disclose the above conditions, 1 to 19 to any person requesting information about your registration status *Closely supervised: The workplace supervisor must supervise the registrant’s day- to-day work in a way prescribed in the relevant condition or undertaking. The workplace supervisor must always be on site and available when the registrant is working. Where the workplace supervisor is unavailable through illness or planned absence, the registrant must not work unless an approved alternative workplace supervisor is in place. The workplace supervisor must review the registrant’s work at least twice a week in one-to-one meetings using case-based discussions. These meetings must focus on all areas of concern identified by the conditions or undertakings. These meetings can be in person or via video conferencing.
