1. She must notify the GDC promptly of any post she accepts for which GDC registration is required and the Commissioning Body on whose Dental Performers List she is included.
2. If employed, she must provide contact details of her employer and allow the GDC to exchange information with her employer or any contracting body for which she provides dental services.
3. She must inform the GDC of any formal disciplinary proceedings taken against her, from the date of this determination.
4. She must inform the GDC if she applies for dental employment outside the UK.

PUBLIC CONDITIONS
10. At any time she is employed, or providing dental services, which require her to be registered with the GDC; she must place herself and remain under the direct supervision* of one or more workplace supervisors nominated by her and agreed by the GDC.
11. She must keep a log of her meetings with her workplace supervisors and provide them for consideration by this Committee, prior to any review hearing.
12. She must seek reports from her workplace supervisors, for consideration by this Committee, prior to any review hearing.
13. She must not engage in single-handed dental practice and must only work at premises where another dental professional or dental professionals are working at the same time as she is working and with whom she has made personal contact before she commences treatment of patients at each session.
14. She must inform within one week the following parties that her registration is subject to the conditions, listed at (1) to (4) and (10) to (13), above:
• Any organisation or person employing or contracting with her to undertake dental work
• Any locum agency or out-of-hours service she is registered with or apply to be registered with (at the time of application).
• Any prospective employer (at the time of application).
• The Commissioning Body in whose Dental Performers List she is included or seeking inclusion (at the time of application).
15. She must permit the GDC to disclose the above conditions, (1) to (4) and (10) to (14) to any person requesting information about her registration status.
*Direct Supervision
The registrant’s work must be directly supervised at all times by a person who is registered with the GDC in their category of the register or above and who must be on site and available at all times. The level of supervision required is equivalent to that of a Foundation Trainee. The registrant may nominate more than one workplace supervisor to be agreed by the GDC. The requirement for direct supervision will be met so long as any one of the nominated and agreed workplace supervisors is on site and available at all times that the registrant is working. The registrant’s work must be reviewed at least twice a week by one or more of the supervisors who have supervised the registrant during that week. These reviews must take place via one to one meetings and case-based discussion. These twice-weekly meetings must be focused on all areas of concern. These meetings can take place virtually or face-to-face.
Registration History:
At a review hearing on 03 April 2025, the Interim Orders Committee continued the conditions on Ms Moazzam's registration for the remainder of the High Court extension. The order was originally imposed on 07 November 2022 for 15 Months. On 25 January 2024, the High Court extended the order for 12 months. On 16 January 2025, the High Court extended the order again for a further 7.5 months. This will be reviewed in 6 months. The conditions on registration are listed on Ms Moazzam's register entry.
