Conditions:
1. You must notify the GDC within 7 days of any professional appointment you accept and provide the contact details of your employer or any organisation for which are you contracted to provide dental services and the Commissioning Body on whose Dental Performers List you are included. 2. You must allow the GDC to exchange information with your employer or any organisation for which you are contracted to provide dental services, or any Workplace reporter referred to in these conditions. 3. From the date that these conditions take effect, you must inform the GDC within seven days of being notified of: a) any formal disciplinary action taken against you b) any NHS investigation c) any regulatory or enforcement action taken against you or a practice for which you are the registered provider d) any complaints, formal or informal, made against you. 4. You must inform the GDC within 7 days if you apply for dental employment outside of the UK. 5. You must inform the GDC, within 7 days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within 7 days of making an application for registration with any overseas regulator or equivalent authority. 6. At any time you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter nominated by you and approved by the GDC. The workplace reporter must be registered with the GDC with at least 1 years’ experience. 10. You must not start/restart work until your proposed workplace reporter has been approved by the GDC.
11. You must provide the workplace reporter with a copy of the determination before they agree to become your workplace reporter. You must forward written evidence of your compliance with this condition to the GDC within 7 days of notifying parties. 12. You must not discuss the GDC investigation with anyone else at your workplace beyond informing relevant parties of the conditions in place on your practice, for example the Workplace Reporter. 13. You must provide reports from your workplace reporter to the GDC every 3 months and at least 14 days prior to any review hearing. 14. In addition to addressing general compliance with the conditions, any complaints received and any other relevant information, you must ensure the report specifically addresses: a) Dates of meetings between the workplace reporter and yourself; b) Confirmation that you are complying with the conditions; c) PRIVATE d) Details of any formal disciplinary action; e) Any other information relevant to your compliance with their conditions. 15. You must inform within 7 days the following parties, that your registration is subject to the conditions, listed at 1-5 and 9 -14 (a), (b), (d) and (e) above, and provide evidence to the GDC that this has been done: a)Any organisation or person employing or contracting with you to undertake dental work; b)Any prospective employer (at the time of application); c) The Commissioning Body or Health Board in whose Dental Performers List you are included or seeking inclusion (at the time of application); d) Every member of the dental team and all employees at the Practice where you work. They must also be informed that they should contact the Workplace Reporter and the GDC immediately if they are concerned about your fitness to practice and your compliance with the conditions for the Workplace Reporter to include within their report. 16. You must permit the GDC to disclose the above conditions 1-5 and 9-14 (a), (b), (d) and (e) and 15 to any person requesting information about your registration status. Workplace reporter: A workplace reporter will provide reports to the GDC on the Registrant’s compliance with conditions or undertakings and will notify the GDC of any concerns or complaints. Practice Committees can also require workplace reporters to report on progress towards remediation in an area or areas of concern.
Registration History:
On 23 June 2026, the Interim Orders Committee imposed conditions on Ms Ghazanfar's registration for a period of 18 months. This will be reviewed in 6 months.
