                1. You must notify the GDC within 7 days of any professional appointment you accept or are currently undertaking and provide the contact details of your employer, any organisation for which you have a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List.
2. You must allow the GDC to exchange information with your employer, any organisation for which you have a contract or arrangement to provide dental services, all the NHS regional teams/Health Boards with whom you have an arrangement.
3. You must inform the GDC within 7 days of any formal or informal disciplinary proceedings taken against you from the day these conditions take effect.
4. You must inform the GDC within 7 days of any complaints made against you from the date these conditions take effect.
5. You must inform the GDC within 7 days if you apply for dental employment outside the UK from the day these conditions take effect.
6. You must inform the GDC if you are registered or apply for registration with any overseas regulator/relevant authority, within 7 days of the date of the application or 7 days from the date of this determination if already registered.
7. Within 7 days of these conditions taking effect, you must agree to the appointment of a chaperone or chaperones, nominated by you and approved by the GDC. Your chaperone(s) must be a registered dentist or DCP. The chaperone(s) must be a different person from the DCP or dentist who is working with you at any given time.
8. You must not start/restart work until your proposed chaperone(s) has been approved by the GDC.
            
        
                9. Within 7 days of these conditions taking effect, you must share a copy of these conditions with your chaperone(s) and provide written evidence to the GDC to demonstrate you have done so.
10. Except in life threatening emergencies, you must not work with female colleagues in a clinical setting, whether a patient is present or not, or have consultations with female patients, without your chaperone(s) present.
11. You must maintain a log of all instances of working with female colleagues in a clinical setting, whether a patient is present or not, and consultations with female patients, which must be signed by your chaperone(s) confirming your chaperone’s(s’) presence throughout the instance or consultation.
12. You must maintain a log detailing every instance where you have worked with female colleagues in a clinical setting, whether a patient is present or not, or have had consultations with female patients, in a life-threatening emergency without your chaperone(s) present.
13. You must provide a copy of the logs referred to at conditions (11) and (12) above to the GDC every 3 months and at least 14 days prior to any review hearing. Alternatively, you must provide a statement confirming there have been no such instances or consultations. The log or statement must be signed by your chaperone(s).
14. At any time, you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter(s) nominated by you and approved by the GDC.
15. You must not start/restart work until your proposed workplace reporter(s) has been approved by the GDC.
16. Your chaperone(s) and the reporter(s) could be the same person.
17. You must provide reports from your reporter(s) to the GDC every 3 months and at least 14 days prior to any review hearing. The reports should include:
- Confirmation of compliance with the conditions
- Details of any complaints/concerns raised
18. You must inform within 7 days the following parties that your registration is subject to the conditions; and provide written evidence to the GDC within 7 days of this being completed.
a) Any organisation or person employing or contracting with him to undertake dental work
b) Any prospective employer (at the time of application); and
c) The Commissioning Body or Health Board in whose Dental Performers List he is included or seeking inclusion (at the time of application.)
19. You must permit the GDC to disclose the above conditions, (1) to (18), to any person requesting information about your registration status.
            
        
        Registration History:
    
            On 18 September 2025, the Interim Orders Committee imposed conditions on Mr Shokry's registration for a period of 12 months.
This will be reviewed in 6 months.
