1) You must notify the GDC within seven days of any professional appointment you accept or are currently undertaking and provide the contact details of your employer, any organisation for which you have a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List. 2) You must allow the GDC to exchange information with your employer, any organisation for which you have a contract or arrangement to provide dental services, all the NHS regional teams/Health Boards with whom you have an arrangement. 3) You must inform the GDC within seven days of any formal or informal disciplinary proceedings taken against you from the day these conditions take effect. 4) You must inform the GDC within seven days of any complaints made against you from the date these conditions take effect. 5) You must inform the GDC within seven days if you apply for dental employment outside the UK from the day these conditions take effect. 6) You must inform the GDC if you are registered or apply for registration with any overseas regulator/relevant authority, within seven days of the date of the application or seven days from the date of this determination if already registered. 7) At any time, you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter(s) nominated by you and approved by the GDC. 8) You must not start/restart work until your proposed workplace reporter(s) has been approved by the GDC. 9) You must provide reports from your reporter(s) to the GDC every three months and at least 14 days prior to any review hearing. The reports should include: a) Confirmation of compliance with the conditions b) Details of any complaints/concerns raised by colleagues, patients and/or members of the public regarding your behaviour and/or conduct 10) You must inform within seven days the following parties that your registration is subject to the conditions; and provide written evidence to the GDC within seven days of this being completed. a) Any organisation or person employing or contracting with him to undertake dental work b) Any prospective employer (at the time of application); and c) The Commissioning Body or Health Board in whose Dental Performers List he is included or seeking inclusion (at the time of application.) 11) You must permit the GDC to disclose the above conditions 1) to 10), to any person requesting information about your registration status.
