Conditions:
1. You must provide the GDC, within seven days, the contact details and arrangements for any appointment you accept or are currently undertaking which requires GDC registration, and allow the GDC to exchange information with your employer or any contracting body for which you provide dental services. 2. You must provide the GDC, within seven days, the contact details of the Commissioning Body on whose Dental Performers List you are included or Local Health Board if in Wales, Scotland or Northern Ireland. 3. From the date that these conditions take effect, you must inform the GDC within seven days of being notified of: i. any formal disciplinary action taken against you ii. any investigation into your practice undertaken by the commissioning body in whose Dental Performers List you are included iii. any regulatory or enforcement action taken against you or a practice for which you are the registered provider iv. any patient complaint received about your clinical practice or conduct at work. 4. You must inform the GDC, within seven days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within seven days of making an application for registration with any overseas regulator or equivalent authority. 5. You must not carry out any of the procedures / treatment listed at condition 6 unless directly supervised* by a workplace supervisor nominated by you and approved by the GDC. The workplace supervisor must be a GDC registered dentist with a minimum of 5 years’ UK experience. You must not start/restart work until your proposed workplace supervisor has been approved by the GDC. 6. You must maintain a log detailing every case where you have undertaken the following: • Basic Periodontal Examinations (BPE) • Root canal treatment (RCT), including RCT aftercare • Diagnosis and treatment of caries • Radiographs, including justification and grading/reporting • Administration of local anaesthetic • Taking pain histories / pain management The logs must be checked and signed by your workplace supervisor on a weekly basis. 7. You must provide a copy of this log to the GDC every 3 months and at least 14 days prior to any review hearing. 8. You must carry out an audit in the following areas of your practice every 3 months: • Record keeping • Payments and claims for treatment • Basic Periodontal Examinations (BPE) • Root canal treatment (RCT), including RCT aftercare • Diagnosis and treatment of caries • Radiographic practice, including justification and grading/reporting • Administration of local anaesthetic • Taking pain histories / pain management. The sample size for the audits should be approved by your workplace supervisor as being proportionate whilst providing a reasonable picture of your work. The audits must be checked and signed by your workplace supervisor. 9. You must provide a copy of this audit to the GDC every 3 months and at least 14 days prior to any review hearing or, alternatively, provide a statement, which has been counter-signed by your workplace supervisor, confirming there have been no such cases. 10. You must provide reports from your workplace supervisor to the GDC every 3 months and at least 14 days prior to any review hearing. The reports should include information about: • your compliance with these conditions. • your clinical performance. • one-to-one meetings and case-based discussions. • any concerns regarding you or your practice. • commentary on whether work has been carried out to an adequate standard and, in particular, whether: o logs and audits include a satisfactory level of detail o adequate records are being kept o BPEs are being carried out satisfactorily o x-rays are being administered properly o caries are being diagnosed and treated satisfactorily o root canal treatment is being carried out to a satisfactory standard.
11. You must stop working immediately if your workplace supervisor directs you to do so, and you must inform the GDC within seven days of receiving this direction. 12. You must not work as a locum or undertake any out-of-hours work or on-call duties. 13. You must inform, within seven days, the following parties that your registration is subject to the conditions listed at 1 to 12: i. Any organisation or person employing you or who has an arrangement with you to undertake dental work. ii. Any professional regulatory body you are registered with, or apply to be registered with (at the time of application). iii. Any prospective employer (at the time of application). iv. The commissioning body in whose Dental Performers List you are included, or seeking inclusion (at the time of application). You must forward written evidence of your compliance with this condition to the GDC within seven days of notifying the relevant parties of your conditions. 14. You must permit the GDC to disclose the above conditions, (1) to (13), to any person requesting information about your registration status. *Directly Supervised: the workplace supervisor must observe the registrant’s day-to-day work, or the particular element(s) of the registrant’s work, as prescribed in the relevant condition or undertaking. The workplace supervisor must always be on site and available to directly observe the specified element(s) when the registrant is working. Where the workplace supervisor is unavailable through illness or planned absence, the registrant must not carry out the specified element(s) of their work unless an approved alternative workplace supervisor is in place. Conditions or undertakings will specify which element(s) of a registrant’s practice to observe.
Registration History:
On 11 March 2025, the Interim Orders Committee imposed conditions on Ms Hanna's registration for a period of 18 months. This will be reviewed in 6 months.
