Conditions:
1. You must provide the GDC, within seven days, the contact details and arrangements for any appointment you accept or are currently undertaking which requires GDC registration, and allow the GDC to exchange information with your employer or any contracting body for which you provide dental services. You must provide the GDC, within seven days, details of any organisation for which you are contracted to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List 2. From the date that these conditions take effect, you must inform the GDC within seven days of being notified of: a. any formal disciplinary action taken against you b. any NHS investigation c. any regulatory or enforcement action taken against you or a practice for which you are the registered provider d. any patient complaint received about your clinical practice or conduct at work 3. You must inform the GDC, within seven days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within seven days of making an application for registration with any overseas regulator or equivalent authority. 4. You must not carry out work with any locum agency or out-of-hours service. Workplace supervisor 5. At any time you are employed to provide dental services which require you to be registered with the GDC, you must remain under the supervision of a workplace supervisor nominated by you and approved by the GDC. The workplace supervisor must be a GDC registered dentist. You must not start/restart work until your proposed workplace supervisor has been approved by the GDC. 6. You must provide reports from your workplace supervisor to the GDC every three months and at least 14 days prior to any review hearing. The workplace supervisor’s report should address the following areas: a. Dates of meetings between you and the workplace supervisor; b. Dates audit was conducted and how it was conducted (see condition 13); c. Any concerns from the audits carried out (see condition 13); d. Confirmation that you are complying with the conditions; e. provide information and/or feedback concerning each of the matters listed under condition 10, commenting on whether or not work has been carried out to an adequate standard; f. Details of any concerns, if any, about your fitness to practise; g. Details of any formal disciplinary action; h. Any other information relevant to your compliance with the conditions. 7. You must present the Workplace Supervisor with a copy of this determination within seven days of the Workplace Supervisor being approved and of any subsequent hearings in relation to this interim order within seven days of the hearing. 8. You must provide the GDC with written evidence of compliance with condition 7 within seven days. 9. You must stop working immediately if your Workplace Supervisor directs you to do so, and you must inform the GDC within seven days of receiving this direction
Logs 10. You must keep a log of every case you have undertaken where a patient (adult or child) has attended treatment for dental trauma and/or where you are considering referral to an external statutory agency and have a record of the following: a. Pretreatment and assessment which will include treatment planning, attainment of informed consent, diagnosis, ensuring treatment given/planned is clinically required and appropriately managed; b. Treatment planning to include explaining treatment options, including their risks and benefits; c. Record keeping throughout all stages of assessment and treatment and include, but not be limited to, justification for diagnosis and reasons treatment plans; d. Radiographs, including grading and justification; e. Alternative approaches/treatments, including explaining their risks and benefits; f. Any referrals to any external statutory agency and the reasons for those referrals. 11. The Workplace Supervisor will require a copy of this log from you every fortnight and must review the contents. The workplace supervisor will sign and date the fortnightly log if they consider the documentation to be satisfactory. 12. You must provide a copy of this log, signed by your workplace supervisor, to the GDC every three months and at least 14 days prior to any review or alternatively, provide a written statement confirming that there have been no such cases during that period also verified by your workplace supervisor. 13. You must allow your workplace supervisor to conduct an audit of your log at least every three months. Workplace Reporter 14. At any time, you are providing dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter nominated by you and approved by the GDC. The workplace reporter shall be a GDC registrant in the same category of register as the registrant or higher and may be the same person as the workplace supervisor under condition 8. You must not start/restart work until your proposed workplace reporter has been approved by the GDC. 15. You must provide reports from your Workplace Reporter to the GDC every three months and at least 14 days prior to any review hearing. The workplace Reporter’s report should address the following areas: Dates of meetings between you and the reporter; a. Confirmation that you are complying with their conditions; b. Details of any concerns, if any, about your fitness to practise; c. Details of any formal disciplinary action; d. To discuss how to have a clear and effective complaints procedure; e. Any other information relevant to your compliance with the conditions. 16. You must allow the Workplace Reporter to exchange information with the GDC and to provide reports to the GDC every three months and at least 14 days prior to any review hearing. 17. You must present the Workplace Reporter with a copy of this determination within seven days of the reporter being approved and provide the GDC with written evidence that you have notified your reporter/a copy of notification to your reporter within seven days. If the Workplace Reporter does not work at the same place as you, they must be able to attend the practice and access any documentation relevant to your conditions.
18. You must inform, within seven days, the following parties that your registration is subject to the conditions listed at 1 to 17: a. Any organisation or person employing you or who has an arrangement with you to undertake dental work. b. Any professional regulatory body you are registered with, or apply to be registered with (at the time of application). c. Any prospective employer (at the time of application). d. The commissioning body in whose Dental Performers List you are included, or seeking inclusion (at the time of application) e. NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List You must forward written evidence of your compliance with this condition to the GDC within seven days of notifying the relevant parties of your conditions *Supervised: the Workplace Supervisor must supervise the registrant’s day-to-day work in a way prescribed in the relevant condition or undertaking. The Workplace Supervisor does not need to work at the same practice as the registrant, but they must be available to provide advice or assistance if the registrant needs it. Where the Workplace Supervisor is unavailable through illness or planned absence, the registrant must not work, unless an approved alternative Workplace Supervisor is in place. The Workplace Supervisor must review the registrant’s work at least once a fortnight in one-to-one meetings and case-based discussions. These meetings must focus on all areas of concern identified by the conditions or undertakings. These meetings should usually be in person. If this is not possible, at least one of every two fortnightly meetings must be in person.

At a review hearing on 2 April 2026, the Interim Orders Committee continued the conditions on Ms Lee's registration for the remainder of the 12 month order.
