Conditions:
1. You must provide the GDC, within seven days, the contact details and arrangements for any appointment you accept or are currently undertaking which requires GDC registration, and allow the GDC to exchange information with your employer or any contracting body for which you provide dental services.
2. From the date that these conditions take effect, you must inform the GDC within seven days of being notified of:
i. any formal disciplinary action taken against you;
ii. any NHS investigation;
iii. any regulatory or enforcement action taken against you or a practice for which you are the registered provider;
iv. any patient complaint received about your clinical practice or conduct at work.
3. You must inform the GDC, within seven days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within seven days of making an application for registration with any overseas regulator or equivalent authority.
4. You must allow the GDC to exchange information with the Practice, your employer, any organisation for which you have a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement.
5. You must not contact patients directly following complaints to the GDC or otherwise unless complying with the practice complaints procedure, where applicable.
6. At any time you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter nominated by you and approved by the GDC.
You must not start/restart work until your proposed workplace reporter has been approved by the GDC.
7. You must provide reports from your workplace reporter to the GDC every three months and at least 14 days prior to any review hearing.
8. At any time you are employed to provide dental services, which require you to be registered with the GDC, you must remain under the supervision* of a workplace supervisor nominated by you and approved by the GDC. The workplace supervisor must be a GDC registered dentist.
You must not start/restart work until your proposed Workplace Supervisor has been approved by the GDC.
9. You must not be involved with any remote orthodontic services except in accordance with the GDC’s statement on ‘direct to consumer’ orthodontic treatment.
10. You must provide reports from your Workplace Supervisor to the GDC every three months and at least 14 days prior to any review hearing about orthodontic treatment, consultations and advice given to patients with which she is involved.
11. You shall complete a log which must be signed by your Workplace Supervisor in respect of the following aspects of orthodontic treatment;
a) Diagnosis assessments;
b) Record keeping;
c) Treatment planning and/or treatment options including discussion of the risks and benefits with patients;
d) Radiographs;
e) Informed consent.
12. You must provide a copy of the log referred on Condition 11 to the GDC every three months and at least 14 days prior to any review hearing.
13. You must keep your professional commitments under review and limit your dental practice in accordance with the Workplace Supervisor’s advice.
14. You must inform, within seven days, the following parties within the UK that your registration is subject to the conditions listed at [1] to [13]:
• Any organisation or person employing you or who has an arrangement with you to undertake dental work;
• Any professional regulatory body you are registered with, or apply to be registered with (at the time of application);
• Any locum agency or out-of-hours service you are registered with or apply to be registered with (at the time of application);
• Any prospective employer (at the time of application);
• The Commissioning body in whose dental Performers List you are included, or seeking inclusion (at the time of application);
• Your reporter; and,
• Your Workplace Supervisor
You must forward written evidence of your compliance with this condition to the GDC within seven days of notifying the relevant UK parties of your conditions.
* Supervised: the workplace supervisor must supervise the registrant’s day-to-day work in a way prescribed in the relevant condition or undertaking. The workplace supervisor does not need to work at the same practice as the registrant, but they must be available to provide advice or assistance if the registrant needs it. Where the workplace supervisor is unavailable through illness or planned absence, the registrant must not work, unless an approved alternative workplace supervisor is in place.
The workplace supervisor must review the registrant’s work at least once a fortnight in one-to-one meetings and case-based discussions. These meetings must focus on all areas of concern identified by the conditions or undertakings. These meetings should usually be in person, where possible.

Conditions:
1. She must notify the GDC within 7 days of any professional appointment she accepts or is currently undertaking and provide the contact details of any branch(es) of the Practice for which she is Director, her employer, any organisation for which she has a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom she has an arrangement, either as a contract holder or by being on their Dental Performers’ List.
2. She must allow the GDC to exchange information with the Practice, her employer, any organisation for which she has a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom she has an arrangement.
3. She must inform the GDC within 7 days of any formal disciplinary proceedings taken against her from the date these conditions take effect.
4. She must inform the GDC within 7 days of any complaints made against her from the date these conditions take effect.
5. She must inform the GDC within 7 days if she applies for dental employment outside the UK.
6. She must inform the GDC if she is registered or applies for registration with any overseas regulator/relevant authority, within 7 days of the date of the application or 7 days from the date of this determination if already registered.
7. At any time she is employed, or providing dental services which require her to be registered with the GDC, she must agree to the appointment of a reporter nominated by her and approved by the GDC. The reporter shall be a GDC registrant.
8. She must allow the reporter to provide reports to the GDC at intervals of not more than 3 months and 14 days prior to any review hearing.
9. At any time she is employed, or providing dental services, which require her to be registered with the GDC, she must place herself and remain under the supervision of a workplace supervisor nominated by herself and agreed by the GDC. Compliance with this condition could be met if the face to face discussions are on a virtual platform.
10. She must not be involved with tooth whitening except in accordance with the GDC position statement on tooth whitening.
11. She must allow her Workplace Supervisor to provide reports to the GDC every 3 months and at least 14 days prior to any review hearing about tooth whitening procedures, consultations and advice given to patients with which she is involved.
12. She shall complete a log which must be signed by her Workplace Supervisor about tooth whitening procedures, consultations and advice given to patients with which she is involved.
13. She must provide a copy of the log referred on Condition 12 to the GDC every 3 months and at least 14 days prior to any review hearing.
14. She must keep her professional commitments under review and limit her dental practice in accordance with the workplace supervisor’s advice.
15. She must inform within 7 days the following parties that his registration is subject to the conditions, listed at 1 to 14 above:
• Any organisation or person employing or contracting with her to undertake dental work;
• Any locum agency or out-of-hours service she is registered with or applies to be registered with (at the time of application);
• Any prospective employer (at the time of application);
• The Commissioning Body or on whose Dental Performers List she is included or seeking inclusion (at the time of application);
• Her reporter; and,
• Her Workplace Supervisor
16. She must permit the GDC to disclose the above conditions 1 to 15 to any person requesting information about her registration status.
Registration History:
At a review hearing on 8 April 2025 the Interim Orders Committee continued the conditions on Ms Naz's registration for the remainder of the High Court extension. The interim order was originally imposed on 1 November 2021 for 18 months. On 19 April 2023 the High Court extended the order for 12 months. On 22 April 2024 the High Court extended the order for a further 9 months. On 22 January 2025 the High Court extended the order for a further 12 months.
