1) You must notify the GDC within seven days of any professional appointment you accept or are currently undertaking and provide the contact details of your employer, any organisation for which you have a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List. 2) You must allow the GDC to exchange information with your employer, any organisation for which you have a contract or arrangement to provide dental services, all the NHS regional teams/Health Boards with whom you have an arrangement. 3) You must inform the GDC within seven days of any formal or informal disciplinary proceedings taken against you from the day these conditions take effect. 4) You must inform the GDC within seven days of any complaints made against you from the date these conditions take effect. 5) You must inform the GDC within seven days if you apply for dental employment outside the UK from the day these conditions take effect. 6) You must inform the GDC if you are registered or applies for registration with any overseas regulator/relevant authority, within seven days of the date of the application or seven days from the date of this determination if already registered. 7) At any time you are providing dental services, which you to be registered with the GDC, you must agree to the appointment of a reporter nominated by you and approved by the GDC. The reporter shall be a GDC registrant in the same category as you (or higher). The reporter shall work in the same practice as you. 8) You must provide a report from the reporter to the GDC every two months and at least 14 days prior to any review hearing. This report should include: a) Confirmation of compliance with these conditions; b) Details of any complaints received and their resolution; c) Any other relevant information. 9) You must allow the GDC to exchange information about your conditions with any reporter. 10) You must refrain from carrying out implant treatment, including ceramic implants. 11) You must refrain from making statements via social media that imply a connection between root canal surgery and Lyme disease, or root canal surgery and inflammatory conditions or that suggest a link between metal amalgam fillings and mercury toxicity. 12) You must also remove any existing online content relating to such statements as stated in Condition 11). 13) You must provide the GDC with a signed statement confirming compliance with Condition 12) within seven days from the day these conditions take effect. 14) You must inform within seven days the following parties that your registration is subject to the conditions, and provide written evidence to the GDC within seven days of this being completed: a) Any organisation or person employing or contracting with you to undertake dental work; b) Any prospective employer (at the time of application); and c) The Commissioning Body or Health Board in whose Dental Performers List you are included or seeking inclusion (at the time of application). 15) You must permit the GDC to disclose Conditions 1) to 13), above, to any person requesting information about your registration status.
