Conditions:
1. The following conditions can only be fulfilled in the UK. 2. She must notify the GDC within 7 days of any post she accepts for which GDC registration is required and the Commissioning Body on whose Dental Performers List she is included. 3. If employed, she must provide contact details of her employer within 7 days and allow the GDC to exchange information with her employer or any contracting body for which she provides dental services. 4. She must inform the GDC within 7 days of any formal disciplinary proceedings taken against her, from the date of this determination. 5. She must inform the GDC within 7 days of any complaints made against her from the date these conditions take effect. 6. She must inform the GDC if she applies for dental employment outside the UK within 7 days of making any application. 7. She must, at all times whilst she is providing dental services, which require her to be registered with the GDC, place herself under the supervision* of a workplace supervisor. The workplace supervisor shall be a GDC registrant in the same category of the register as the registrant or higher. She must not undertake any clinical work until a workplace supervisor, nominated by her, has been agreed by the GDC. 8. She must provide the workplace supervisor with a copy of this determination immediately after the supervisor has been approved by the GDC. Evidence that this information has been provided to the workplace supervisor must be forwarded to the GDC within 7 days of disclosure. 9. She must allow the GDC to exchange information with her workplace supervisor. 10. She must provide a report from her workplace supervisor to the GDC every three months and at least 14 days prior to any review. The report will address the following area: • Cross infection control and decontamination procedures 11. She shall maintain logs of: • Cross infection control and decontamination procedures 12. She must work at all times with a dental nurse who is registered with the GDC. 13. The logs must be countersigned by the dental nurse referred to at condition (12) above at the end of each treatment session. These logs must then be provided to her workplace supervisor prior to her fortnightly meeting. 14. She must provide copies of these logs to the GDC every 3 months and at least 14 days prior to any review hearing. 15. She must inform the following parties within 1 week of the date of this determination that her registration is subject to the conditions listed at (1) to (14) above and provide written evidence of her compliance with this condition to the GDC within 7 days. i. Any organisation or person employing or contracting with her to undertake dental work. ii. Any locum agency or out-of-hours service she is registered with or applies to be registered with (at the time of application). iii. Any prospective employer (at the time of application). iv. The Commissioning Body in whose Dental Performers List she is included, or seeking inclusion (at the time of application). 16. She must permit the GDC to disclose the above conditions, (1) to (15), to any person requesting information about her registration status. *Supervised The registrant’s day to day work must be supervised by a person who is registered with the GDC in their category of the register or above. The supervisor need not work at the same practice as the registrant, but must make themselves available to provide advice or assistance should they be required. The registrant’s work must be reviewed at least once fortnightly by the supervisor via one-to-one meetings and case-based discussion. These fortnightly meetings must be focused on all areas of concern identified by the conditions/undertakings. These meetings may be held virtually.

1. She must notify the GDC within 7 days of any post she accepts for which GDC registration is required and the Commissioning Body on whose Dental Performers List she is included. 2. If employed, she must provide the full name and contact details of her employer to the GDC within 7 days of this determination and allow the GDC to exchange information with her employer or any contracting body for which she provides dental services. 3. She must inform the GDC within 7 days of any formal disciplinary proceedings taken against her, from the date of this determination. 4. She must inform the GDC within 7 days of any complaint made against her, from the date of this determination. 5. She must inform the GDC if she applies for dental employment outside the UK, within 7 days of such an application being made. 6. At any time she is providing clinical treatment, she must have another GDC Registrant present in the treatment room to provide chairside support and act as a chaperone. The GDC Registrant/chaperone should be nominated by her by providing the full name, contact details and GDC number, to be agreed by the GDC. 7. At any time she is providing dental services, which require her to be registered with the GDC, she must agree to the appointment of a Reporter who works at the same premises. The Reporter shall be a GDC registered dentist, nominated by her and approved by the GDC. 8. She must allow the Reporter to provide reports to the GDC monthly, and at least 14 days prior to any review. The reports will include details of the compliance with these conditions, whether there have been any concerns or complaints raised and confirmation that the chaperone requirement at condition 6 is being complied with. 9. At any time she is employed, or providing dental services, which requires her to be registered with the GDC, she must place herself and remain under the *supervision of a Workplace Supervisor, who is registered in the same category or higher, nominated by her, and agreed by the GDC. The role of the Workplace Supervisor and the Reporter can be merged. 10. She must not return to work until the Workplace Supervisor has been approved by the GDC. 11. She must allow the Workplace Supervisor to provide reports to the GDC monthly, and at least 14 days prior to any review. The reports will include details of the discussions and any action points resulting from the one-to-one meetings and cover (including but not limited to) the following areas of clinical practice: • Record Keeping • Pre-treatment Assessment • Risks and benefits of treatment • Treatment Planning • Providing treatment options and associated costs • Informed consent • Composite Bonding • Recognising when a patient wants to stop/have a break • Medical Histories • Refund procedures including reasoning for approval and/or rejection • Complaints Handling/Management • Working with a Chaperone/Chairside Support 12. She must provide the Workplace Supervisor with a copy of the determination immediately after the Supervisor has been approved by the GDC. Written evidence that this information has been provided to the Workplace Supervisor must be forwarded to the GDC within 7 days of disclosure. 13. She must allow the Workplace Supervisor and the GDC to exchange information. 14. She must complete and submit a log detailing the assessment/treatment that has been carried out for every case she undertakes, covering the relevant areas as set out in condition 11, approved and signed by her Workplace Supervisor and her nominated chaperone, to the GDC every month and at least 14 days prior to any review hearing.
15. She must inform within 7 days the following parties that her registration is subject to the conditions, listed at 1 to 14 above, and provide written evidence to the GDC that this has been done: a. Any organisation or person employing or contracting with her to undertake dental work; b. Any prospective employer (at the time of application); c. Any locum agency or out-of-hours service she is registered with or applies to be registered with (at the time of application). d. The Commissioning Body or Health Board in whose Dental Performers List she is included or seeking inclusion (at the time of application); e. The nominated chaperone providing chairside support. They must also be informed that they should contact the Reporter / Workplace Supervisor and the GDC immediately if they are concerned about her fitness to practise and/or her compliance with the conditions for the Reporter / Workplace supervisor to include within their report. 16. She must permit the GDC to disclose the conditions, listed at 1 to 15 above, to any person requesting information about her registration status. *Supervision The workplace supervisor must supervise the registrant’s day-to-day work in a way prescribed in the relevant condition or undertaking. The workplace supervisor does not need to work at the same practice as the registrant, but they must be available to provide advice or assistance if the registrant needs it. Where the workplace supervisor is unavailable through illness or planned absence, the registrant must not work, unless an approved alternative workplace supervisor is in place. The workplace supervisor must review the registrant’s work at least once a fortnight in one-to-one meetings and case-based discussions. These meetings must focus on all areas of concern identified by the conditions or undertakings. These meetings should usually be in person. If this is not possible, at least one of every two fortnightly meetings must be in person.
Registration History:
At a review hearing on 11 February 2025 the Interim Orders Committee continued the conditions on Ms Horri's registration for the remainder of the 18 month order. The order was originally imposed from 21 March 2024. This will be reviewed in 6 months. Ms Horri is currently subject to two interim orders under separate cases. At a review hearing on 27 January 2025, the Interim Orders Committee continued the conditions on Ms Horri's registration for the remainder of the High Court extension. The interim order was originally imposed on 15 May 2023 for 12 months. On 13 May 2024 the High Court extended the order for 6 months. On 11 November 2024 the High Court extended the order for 12 months. This will be reviewed within 6 months. Ms Horri is currently subject to two interim orders under separate cases.
