Conditions:
Dentists Register:
1. You must notify the GDC within 7 days of any professional appointment you accept and provide the contact details of your employer or any organisation for which you are contracted to provide dental services and the Commissioning Body on whose Dental Performers List you are included or Local Health Board if in Wales, Scotland or Northern Ireland.
2. You must allow the GDC to exchange information with your employer or any organisation for which you are contracted to provide dental services, or any Workplace supervisor referred to in these conditions.
3. You must inform the GDC of any formal disciplinary proceedings taken against you, within 7 days, from the date these conditions take effect.
4. You must inform the GDC of any complaints made against you, within 7 days, from the date these conditions take effect.
5. You must inform the GDC if you apply for dental employment outside the UK, within 7 days, from the date these conditions take effect.
6. At any time you are employed, or providing dental services, which require you to be registered with the GDC, you must place yourself and remain under the close* supervision of a workplace supervisor nominated by you and agreed by the GDC. The workplace supervisor shall be a GDC registrant in the same category of the register as the registrant or higher.
7. You must not start or restart work until your workplace supervisor has been approved by the GDC.
8. You must present the workplace supervisor with a copy of this determination. You must provide written evidence to the GDC of having done so within 7 days of the workplace supervisor being approved.
9. You must maintain logs of the following;
• Examination, to include assessment, diagnosis and obtaining of medical histories;
• Record keeping;
• Radiographic practice;
• Anaesthetic practice;
• Restoration technique;
• Hygiene and cross infection control;
• Consultation with patients, to include discussion of risks/benefits, treatment planning, communicating treatment plans to patients and obtaining informed consent.
10. The logs must be signed by your workplace supervisor.
11. You must provide a copy of the logs to the GDC at intervals of two months and 14 days prior to any review hearing or alternatively confirm that there have been no such cases.
12. You must provide your workplace supervisor’s reports to the GDC at intervals of two months and at least 14 days prior to any review; these reports are to comment on the following:
• Examination;
• Record keeping;
• Radiographic practice;
• Anaesthetic practice;
• Restoration technique;
• Hygiene and cross infection control;
• Assessment and diagnosis;
• Consultation with patients including discussion of risks/benefits and obtaining medical history;
• Treatment planning, and communicating treatment plans to patients;
• Obtaining informed consent;
• Full detail of the discussions between you and your workplace supervisor;
• A review of the logs and any action points resulting from the meetings with your supervisor.
13. You must keep your professional commitments under review and limit your dental practice in accordance with your workplace supervisor’s advice.
14. You must stop working immediately if your workplace supervisor directs you to do so, and you must inform the GDC within seven days of receiving this direction.
15. You must not work as a locum or undertake any out-of-hours work or on-call duties.
16. You must inform within 1 week the following parties that your registration is subject to the conditions listed at 1 to 14 above and provide written evidence to the GDC within 7 days of having done so:
• Any organisation or person employing or contracting with you to undertake dental work.
• Any prospective employer (at the time of application)
• The Commissioning Body in whose Dental Performers List you are included, or seeking inclusion (at the time of application)
17. You must permit the GDC to disclose the above conditions to any person requesting information about your registration status.
*Close Supervision
Closely supervised
The registrant’s day to day work must be supervised by a person who is registered with the GDC in their category of the register or above and who must be on site and available at all times. As a minimum, the registrant’s work must be reviewed at least twice a week by the supervisor via one to one meetings and case-based discussion. These bi-weekly meetings must be focused on all areas of concern identified by the conditions/undertakings. These meetings must take place face to face.
Registration History:
On 12 March 2025 the Interim Orders Committee imposed conditions on Ms Shiva's registration for a period of 15 months.
This will be reviewed in 6 months.
