1. You must notify the GDC within 7 days of any professional appointment you accept or are currently undertaking and provide the contact details of your employer, any organisation for which you have a contract or arrangement to provide dental services and all the NHS regional teams/Health Boards with whom you have an arrangement, either as a contract holder or by being on their Dental Performers’ List. 2. You must allow the GDC to exchange information with your employer, any organisation for which you have a contract or arrangement to provide dental services, all the NHS regional teams/Health Boards with whom you have an arrangement. 3. You must inform the GDC within 7 days of any formal or informal disciplinary proceedings taken against you from the day these conditions take effect. 4. You must inform the GDC within 7 days of any complaints made against you from the date these conditions take effect. 5. You must inform the GDC within 7 days if you apply for dental employment outside the UK from the day these conditions take effect. 6. You must inform the GDC if you are registered or apply for registration with any overseas regulator/relevant authority, within 7 days of the date of the application or 7 days from the date of this determination if already registered. 7. At any time you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter nominated by you and approved by the GDC. 8. You must not start/restart work until your proposed workplace reporter has been approved by the GDC. 9. You must share a copy of these conditions with your workplace reporter and provide written evidence to the GDC to demonstrate you have done so. 10. You must meet with your workplace reporter once every month and discuss any concerns or complaints that may have arisen. 11. You must provide reports from your workplace reporter to the GDC every 3 months and at least 14 days prior to any review hearing. 12. You must inform within 7 days the following parties that your registration is subject to the conditions; and provide written evidence to the GDC within 7 days of this being completed: a) Any organisation or person employing or contracting with you to undertake dental work b) Any prospective employer (at the time of application); and c) The Commissioning Body or Health Board in whose Dental Performers List you are included or seeking inclusion (at the time of application.) 13. You must permit the GDC to disclose the above conditions, 1 to 12, to any person requesting information about your registration status.
