1. You must notify the GDC within 7 days of any professional appointment you accept and provide the contact details of your employer or any organisation for which you are contracted to provide dental services and the Commissioning Body on whose Dental Performers List you are included or Local Health Board if in Wales, Scotland or Northern Ireland. 2. You must allow the GDC to exchange information with your employer or any organisation for which you are contracted to provide dental services, or any Workplace supervisor referred to in these conditions. 3. You must inform the GDC of any formal disciplinary proceedings taken against you, within 7 days, from the date these conditions take effect. 4. You must inform the GDC of any complaints made against you, within 7 days from the date these conditions take effect. 5. You must inform the GDC if you apply for dental employment outside the UK, within 7 days from the date these conditions take effect. 6. At any time you are employed, or providing dental services, which require you to be registered with the GDC, you must place yourself and remain under the close* supervision of a workplace supervisor nominated by you and agreed by the GDC. The workplace supervisor shall be a GDC registrant in the same category of the register as the registrant or higher. 7. You must not start or restart work until your workplace supervisor has been approved by the GDC. 8. You must present the workplace supervisor with a copy of this determination. You must provide written evidence to the GDC of having done so within 7 days of the workplace supervisor being approved. 9. You must provide your workplace supervisor’s reports to the GDC at intervals of three months and at least 14 days prior to any review; these reports are to comment on the following: • Record keeping, • Assessments and diagnosis, • Radiographic practice, • Treatment planning, • Informed Consent, • Standard of treatment, • Data handling, • Complaints handling, • A summary of discussions and any action points resulting from the meetings with your supervisor. 10. You must maintain logs of the following: • Record keeping, • Assessments and diagnosis, • Radiographic practice, • Treatment planning, • Informed Consent, • Standard of treatment, • Data handling, • Complaints handling. 11. The logs must be signed by your workplace supervisor. 12. You must provide a copy of the logs to the GDC at intervals of not more than 3 months and 14 days prior to any review hearing, or alternatively confirm that there have been no such cases. 13. You must keep your professional commitments under review and limit your dental practice in accordance with your workplace supervisor’s advice.

14. You must not work as a locum or undertake any out-of-hours work or on-call duties. 15. You must inform within 1 week the following parties that your registration is subject to the conditions listed at 1 to 14 above and provide evidence of having done so: • Any organisation or person employing or contracting with you to undertake dental work. • Any prospective employer (at the time of application) • The Commissioning Body in whose Dental Performers List you are included, or seeking inclusion (at the time of application) 16. You must permit the GDC to disclose the above conditions to any person requesting information about your registration status. * Close supervision The registrant’s day to day work must be supervised by a person who is registered with the GDC in their category of the register or above and who must be on site and available at all times. As a minimum, the registrant’s work must be reviewed at least twice a week by the supervisor via one-to-one meetings and case-based discussion. These bi-weekly meetings must be focused on all areas of concern identified by the conditions/undertakings. These meetings must take place face to face.
