Conditions:
1. He must notify the GDC promptly and within 7 days of any professional appointment he accepts and provide the contact details of his employer or any organisation for which he is contracted to provide dental services and the Commissioning Body on whose Dental Performers List he is included or Local Health Board if in Wales, Scotland or Northern Ireland.
2. He must allow the GDC to exchange information with his employer or any organisation for which he is contracted to provide dental services, or workplace supervisor referred to in these conditions.
3. He must inform the GDC within 7 days of any formal or informal disciplinary proceedings taken against him, from the date of this determination.
4. He must inform the GDC within 7 days of any complaint made against him, from the date of this determination.
5. He must inform the GDC if he applies for dental employment outside the UK, within 7 days of such an application being made.
6. At any time, he is employed, or providing dental services, which require him to be registered with the GDC; he must agree to the appointment of a workplace supervisor nominated by himself and approved by the GDC. The level of supervision will be “closely supervised*” as defined at the end of these condition. The workplace supervisor shall be a GDC registrant in the same category as the Registrant (or higher). The workplace supervisor must work in the same practice as the Registrant.
7. He must not start or restart work until his workplace supervisor has been approved by the GDC.
8. He must present the workplace supervisor with a copy of this determination. Evidence that the determination has been provided to the workplace supervisor must be forwarded to the GDC within 7 days of being approved by the GDC.
9. He must allow his workplace supervisor to provide reports to the GDC at intervals of not more than 3 months and at least 14 days prior to any review. The reports will include details of the discussions and any action points resulting from the one-to-one meetings and address concerns including but not limited to:
• Treatment planning;
• Informed consent;
• Root canal treatment;
• Record keeping;
• Radiographic Practice
10. He shall maintain logs of:
• Treatment planning;
• Informed consent;
• Root canal treatment;
• Record keeping;
• Radiographic Practice.
11. The logs must be signed by his workplace supervisor. He must provide copies of these logs to the GDC every 3 months and at least 14 days prior to any review hearing.
12. He must allow the GDC to exchange information with his workplace supervisor.
13. He must not engage in single-handed dental practice**.
14. He must not provide dental services work as a locum or out-of-hours.
15. He must inform within 7 days the following parties that his registration is subject to the conditions, listed at 1 to 14, above:
a. Any organisation or person employing or contracting with him to undertake dental work;
b. Any prospective employer (at the time of application); and
c. The Commissioning Body or Health Board on whose Dental Performers List he is included or seeking inclusion (at the time of application).
16. Evidence that these parties have been informed that his registration is subject to conditions must be forwarded to the GDC within 7 days of disclosure.
17. He must permit the GDC to disclose the above conditions (1) to (16), to any person requesting information about his registration status.
*Closely supervised: The registrant’s day to day work must be supervised by a person who is registered with the GDC in their category of the register or above and who must be on site and available at all times. As a minimum, the registrant’s work must be reviewed at least twice a week by the supervisor via one to one meetings and case- based discussion. These bi-weekly meetings must be focused on all areas of concern identified by the conditions/undertakings. These meetings must take place face to face.
**Single-handed dental practice
A dentist operating as a sole practitioner without the support of other dentists is working single-handedly. If this is the case, the registrant may not be safe to practise without daily contact from a supervisor or without having the option of seeking assistance from a supervisor on site. In those circumstances it may be appropriate to deny a registrant the right to work single-handedly, for the protection of the public.

Registration History:
At a review hearing on 31 October 2024, the Interim Orders Committee revoked the conditions previously imposed on Mr Khan's registration and replaced them with an order for suspension for the remainder of the 18 month order. On 18 December 2024 the High Court extended the order for 12 months. This will be reviewed within 3 months.
At a review hearing on 20 August 2024, the Interim Orders Committee continued the conditions on Mr Khan's registration for the remainder of the 15 month order. The order was originally imposed on 22 September 2023. On 10 December 2024 the High Court extended the order for 12 months. This will be reviewed within 6 months.
