Conditions:
1. You must provide the GDC, within seven days, the contact details and arrangements for any appointment you accept or are currently undertaking which requires GDC registration and allow the GDC to exchange information with your employer or any contracting body for which you provide dental services. 2. You must also provide the GDC, within seven days, the contact details for the commissioning body or Health Board in whose Dental Performers List you are included, or seeking inclusion (at the time of application). 3. You must allow the GDC to exchange information with your employer or any organisation for which you are contracted to provide dental services. 4. From the date that these conditions take effect, you must inform the GDC within seven days of being notified of: i. any formal disciplinary action taken against you ii. any NHS investigation iii. any regulatory or enforcement action taken against you or a practice for which you are the registered provider iv. any patient complaint received about your clinical practice or conduct at work 5. You must inform the GDC, within seven days of these conditions taking effect, if you are registered with any overseas regulator (or equivalent authority) or within seven days of making an application for registration with any overseas regulator or equivalent authority. 6. You must not carry out work with any locum agency or out-of-hours service. 7. You must inform the GDC within 7 days if you apply for employment to provide dental services outside of the UK. 8. At any time, you are employed to provide dental services which require you to be registered with the GDC, you must remain under the direct supervision* of a workplace supervisor nominated by you and approved by the GDC. The workplace supervisor must be a GDC registrant in the same category of the register as the registrant or higher with at least five years UK experience. You cannot restart/start work without an approved workplace supervisor in place 9. You must stop working immediately if your workplace supervisor directs you to do so, and you must inform the GDC within seven days of receiving this direction 10. At any time, you are employed to provide dental services which require you to be registered with the GDC, you must agree to the appointment of a workplace reporter**in same category of the register or higher than the registrant. The workplace reporter can be nominated by you and must be approved by the GDC. You must not start/restart work until your proposed workplace reporter has been approved by the GDC. 11. You must provide reports from your workplace reporter to the GDC every three months and at least 14 days prior to any review hearing. The reports must include the following: a. Dates of meetings between the registrant and the reporter b. Confirmation that the registrant is complying with their conditions c. Details of any concerns about the registrant’s fitness to practise d. Details of any issues with behaviour in relation to colleagues 12. You must carry out audits in the following areas of your practice • Record keeping (throughout all stages of assessment and treatment). • Pretreatment and assessment which will include treatment planning, attainment of informed consent, diagnosis, ensuring treatment given/planned is clinically required and appropriately managed (such as patient’s pain) • Management of patients - demonstrating duty of candour, professional behaviour and being honest about any failings during clinical treatment. • The standard of actual treatment including crown treatment, root canal treatment, restorations, and bridgework. • Radiographic practices • Sedation practices • Orthodontic treatment • Clinical responsibility- such as adhering to health and safety requirements, using the correct equipment, prescribing antibiotics with clinical justification, undertaking routine examinations between treatments and professional conduct
13. You must provide a copy of these audits, signed by your workplace supervisor, to the GDC every 3 months and at least 14 days prior to any review or alternatively, provide a written statement confirming that there have been no such cases during that period also verified by your workplace supervisor. 14. You must provide reports from your workplace supervisor to the GDC every three months and at least 14 days prior to any review hearing. The workplace supervisor should provide information and/or feedback concerning your compliance with these conditions generally, and particularly the areas of practice listed at condition 12 15. You must not be responsible for the management/administration of any dental practice 16. You must inform, within 7 days, the following parties that your registration is subject to the conditions listed at 1 to 15: • Any organisation or person employing you or who has an arrangement with you to undertake dental work. • Any professional regulatory body you are registered with or apply to be registered with (at the time of application). • Any prospective employer (at the time of application). • The commissioning body or Health Board in whose Dental Performers List you are included or seeking inclusion (at the time of application). 17. You must permit the GDC to disclose the above conditions 1 to 16 to any person requesting information about your registration status. *Directly Supervised: the workplace supervisor must observe the registrant’s day-to-day work, or the particular element(s) of the registrant’s work, as prescribed in the relevant condition or undertaking. The workplace supervisor must always be on site and available to directly observe the specified element(s) when the registrant is working. Where the workplace supervisor is unavailable through illness or planned absence, the registrant must not carry out the specified element(s) of their work unless an approved alternative workplace supervisor is in place. Conditions or undertakings will specify which element(s) of a registrant’s practice to observe. **Workplace reporter A workplace reporter will provide reports to the GDC on the registrant’s compliance with conditions or undertakings and will notify the GDC of any concerns or complaints. Practice committees can also require workplace reporters to report on progress towards remediation in an area or areas of concern. The GDC must approve the workplace reporter before a registrant can return to work. The registrant or their representative will need to provide the workplace reporter’s full name and contact details, current CV and a signed copy of the ‘Workplace reporter agreement form’. The workplace reporter and the workplace supervisor (see below for definition) can be the same person. The workplace reporter must agree to provide reports to the GDC at the intervals specified in the registrant’s conditions or undertakings. They can use a GDC template to meet this requirement. All reports must include: • Dates of meetings between the workplace reporter and the registrant. • Confirmation that the registrant is complying with their conditions or undertakings. • Details of any patient complaints received and the outcome of these complaints. • Details of any concerns raised in the workplace about the registrant’s fitness to practise. • Details of any formal disciplinary action. • Details of the registrant’s progress on remedial activities. • Any other information relevant to the registrant’s compliance with their conditions or undertakings. If the workplace reporter does not work at the same place as the registrant, they must be able to attend the practice and access any documentation relevant to the registrant’s conditions or undertakings. Sometimes, the practice committee, IOC or case examiner may specify that the workplace reporter must always be on premises with the registrant.
