1. You must notify the GDC within 7 days of any dental practices of which you are the owner or part owner, for any post you accept for which GDC registration is required, and the Commissioning Body on whose Dental Performers List you are included. 2. If employed, you must provide contact details of your employer to the GDC within 7 days of this determination and allow the GDC to exchange information with your employer or any contracting body for which you provide dental services. 3. You must inform the GDC within 7 days of any formal disciplinary proceedings taken against you, from the date of this determination. 4. You must inform the GDC within 7 days of any complaint made against you, from the date of this determination. 5. You must inform the GDC if you apply for dental employment outside the UK, within 7 days of such an application being made. 6. You must complete and submit an audit and records of ten cases you undertake which relates to the clinical areas set out below, and provide these to the GDC every month and at least 14 days prior to any review hearing. • Medical histories. • Assessment of occlusion. • Appropriateness of orthodontic treatment including periodontal examination prior to treatment. • Radiographic reporting. • Discussing risks and benefits of treatment with patients. • Written consent. • Justification of treatment including extractions. • Appropriate prescriptions to Orthodontic Therapists and patient management. • Appropriate use of appointment times. • Managing patient complaints. 7. You must inform within 7 days the following parties that your registration is subject to the conditions, listed at 1 to 6 above, and provide written evidence to the GDC that this has been done: a. Any organisation or person employing or contracting with you to undertake dental work; b. any locum agency or out-of-hours service you are registered with or applies to be registered with (at the time of application); c. Any prospective employer (at the time of application); d. any Commissioning Body in whose Dental Performers List you are included or seeking inclusion (at the time of application). 8. You must permit the GDC to disclose the conditions, listed at 1 to 7 above, to any person requesting information about your registration status.
