Conditions:
1. He must within 7 days notify the GDC of any professional appointment he accepts which requires GDC registration and provide the contact details of his employer or any organisation for which he is contracted to provide dental services and details of the Commissioning Body on whose Dental Performers List he is included. 2. If employed, he must provide contact details of his employer and allow the GDC to exchange information with his employer or any contracting body for which he is contracted to provides dental services. 3. He must inform the GDC within 7 days of any formal or informal disciplinary proceedings taken against him, from the date of this determination. 4. He must inform the GDC within 7 days of any complaints made against him from the date these conditions take effect. 5. He must inform the GDC within 7 days from the date of application, if he applies for dental employment outside the UK. 6. At any time he is carrying out dental implant work; he must be supervised* by a workplace supervisor nominated by him and agreed by the GDC. The workplace supervisor shall be a GDC registrant in the same category of the register as the registrant or higher. 7. He must present the workplace supervisor with a copy of this determination within 7 days of the workplace supervisor being approved. He must provide the GDC with a copy of the notification to his workplace supervisor within 7 days. 8. He must not start or restart work until his workplace supervisor has been approved by the GDC. 9. He must permit the GDC and the workplace supervisor to exchange information. 10. He must maintain a log of all implant cases he has undertaken, which must be signed off by the workplace supervisor covering the following: • Treatment planning; • Communication with patients in relation to risks, benefits, possible complications and likely costs and timescales; • Consent. 11. He must meet every two weeks with his workplace supervisor, (this can be done remotely) and conduct work based reflective practice relating to implant cases he has undertaken and discuss any upcoming implant cases including the matters listed at condition 10. 12. He must provide a report from his workplace supervisor commenting on the areas listed in condition 11 above and the logs to the GDC every three months and, at least 14 days in advance of any review hearing. 13. At any time, he is employed, or providing dental services, which require him to be registered with the GDC; he must agree to the appointment of a reporter nominated by him and approved by the GDC. The reporter shall be a GDC registrant in the same category as the registrant (or higher). The reporter shall work in the same practice as the Registrant. 14. He must not start or restart work until his reporter has been approved by the GDC. 15. He must present the reporter with a copy of this determination within 7 days of the reporter being approved. He must provide the GDC with a copy of the notification to his workplace supervisor within 7 days. 16. He must provide a report from the reporter to the GDC every three months and, at least 14 days in advance of any review hearing. This report should include: • Confirmation of compliance from the reporter; • Confirmation of any complaints (formal or informal) received relating to implants; • Any other relevant information. 17. He must allow the GDC to exchange information with his employer or any organisation for which he is contracted to provide dental services referred to in these conditions.
18. He must inform within 7 days, the following parties that his registration is subject to the conditions, listed at (1) to (17), above: • Any organisation or person employing or contracting with him to undertake dental work. • Any prospective employer (at the time of application). • The Commissioning Body in whose Dental Performers List he is included, or seeking inclusion (at the time of application) He must provide evidence to the GDC that he has done so. 19. He must permit the GDC to disclose the above conditions, (1) to (18), to any person requesting information about his registration status. *Supervised The registrant’s implant work must be supervised by a person who is registered with the GDC in their category of the register or above. The supervisor need not work at the same practice as the registrant but must make themselves available to provide advice or assistance should they be required. The registrant’s work must be reviewed at least once fortnightly by the supervisor via one-to-one meetings and case-based discussion. These fortnightly meetings must be focused on all areas of concern identified by the conditions/undertakings.
Registration History:
At a review hearing on 24 April 2024 the Interim Orders Committee continued the conditions on Mr Farrokhe's registration for the remainder of the 15 months order. The order was originally imposed on 11 May 2023. On 23 July 2024 the High Court extended the order for 9 months and 19 days. This will be reviewed in 6 months.
