1. He must notify the GDC promptly, within seven days of any post he accepts for which GDC registration is required and the Commissioning Body on whose Dental Performers List he is included. 2. If employed, he must provide contact details of his employer to the GDC within seven days and allow the GDC to exchange information with his employer or any contracting body for which he provides dental services. 3. He must inform the GDC within seven days of any formal or informal disciplinary proceedings taken against him, from the date of this determination. 4. He must inform the GDC within seven days of any complaints made against him from the date of this determination. 5. He must inform the GDC within seven days from the date of application, if he applies for dental employment outside the UK. 6. At any times he is in the presence of a female patient or female staff member in any surgery, treatment room, office or staff room, he must have a chaperone with him. He must provide the name and full contact details of any proposed chaperone(s) to the GDC within seven days. The chaperone must be a healthcare professional (but does not have to be registered with a professional body) and is over the age of 18 nd must be approved by the GDC. 7. He must allow the GDC to exchange information about his conditions with any chaperone. 8. He must ensure that every member of the dental team or employee at the practice(s) where he works is notified of the conditions. He must permit other members of the dental team or employee(s) at the practice(s) where he works and/or his chaperone to contact the GDC immediately if they are concerned about his fitness to practise and/or compliance with these conditions. 9. At any time he is providing dental services, which require him to be registered with the GDC, he must agree to the appointment of a reporter nominated by him and approved by the GDC. The reporter shall be a GDC registrant but should not be a family member. The reporter may but need not work in the same practice as the registrant. The registrant must make members of the dental team aware that they could notify the reporter of any concerns about his fitness to practise and/or compliance with these conditions. 10. The chaperone and the reporter could be the same person. 11. He must provide a report from the reporter to the GDC every two months and at least 14 days prior to any review hearing. This report should include: • Confirmation of compliance from the chaperone(s) with condition 6 above; • Confirmation of compliance with these conditions; • Confirmation of any complaints received; • Any other relevant information. 12. He must allow the GDC to exchange information about his conditions with any reporter. 13. He shall not contact a female patient or work colleague outside their working environment other than in relation to clinical or practice matters. 14. He must not provide dental services work as a locum or out-of-hours except at his own practice. 15. He must inform within one week the following parties that his registration is subject to the conditions, listed at (1) to (14), above: a) Any organisation or person employing or contracting with him to undertake dental work; b) Any prospective employer (at the time of application); c) All members of staff at his place of work; and, d) Any Commissioning Body in whose Dental Performers List he is included, or seeking inclusion (at the time of application). 16. He must permit the GDC to disclose the above conditions, (1) to (15), to any person requesting information about his registration status.
